Medical Cannabis
in Frankfurt
A comprehensive guide to legally prescribed medical cannabis, therapeutic assessments, pharmacies, and strict separation from recreational use.
Medical vs. Recreational Cannabis
In Germany, medical cannabis is governed entirely separately from recreational use. While recreational cannabis falls under the Consumer Cannabis Act (KCanG), medical cannabis is strictly regulated by the Medical Cannabis Act (MedCanG).
Medical cannabis is a prescribed therapeutic treatment. It includes standardized cannabis flowers, specialized extracts, and specific medications containing cannabinoids (such as Dronabinol). It is not a loophole for recreational consumption.
Who Can Prescribe Medical Cannabis?
Since the legal reforms, cannabis is no longer classified strictly as a narcotic (Betäubungsmittel) in the same tier as heavy opioids, meaning it can now be prescribed on a standard prescription (E-Rezept) rather than a specialized narcotics prescription (BtM-Rezept).
Licensed Doctors
Any licensed physician in Germany (general practitioners, neurologists, pain specialists, psychiatrists) is legally permitted to prescribe medical cannabis. Note: Dentists and veterinarians are excluded.
The Medical Assessment
A doctor will only prescribe cannabis following a thorough medical diagnosis. It is typically considered when standard treatments have failed, have severe side effects, or when the physician determines a high likelihood of positive therapeutic outcomes.
Telemedicine & Online Prescriptions
While telemedicine platforms operate legally in Germany, a prescription is never guaranteed. Legitimate platforms require detailed medical history reviews, questionnaires, and video consultations. We strictly advise against and do not link to any services offering "easy doctors" or automatic prescriptions without proper medical scrutiny.
Pharmacies & Health Insurance
Dispensing via Pharmacies (Apotheken)
Medical cannabis must be obtained through a licensed pharmacy. Many local pharmacies in Frankfurt stock or can order standard pharmaceutical cannabis within a day. Several specialized online pharmacies also serve Frankfurt patients, ensuring quality control and exact strain standardization.
Health Insurance Coverage (Kostenübernahme)
Having a prescription does not guarantee that your statutory health insurance (gesetzliche Krankenkasse) will pay for it. Patients often start as private payers.
To have the costs covered, the prescribing doctor must submit a formal application to the health insurance provider. Coverage is generally only approved for severe, chronic conditions (e.g., chronic pain, severe spasticity, Tourette syndrome) where standard therapies have been exhausted.
Driving & International Travel
Medical Cannabis and Driving
Patients who consume cannabis under a valid doctor's prescription are generally exempt from the strict 3.5 ng/ml recreational THC blood limit, provided they are taking the medication exactly as prescribed and are not impaired.
However, if a patient exhibits driving deficits (impairment), they face the same severe criminal penalties (DUI) as anyone else. Doctors typically advise a period of adaptation where the patient should not drive.
Travelling with Medication
Taking medical cannabis outside of Germany requires strict adherence to international treaties. Within the Schengen Area, patients must carry a specific certificate (pursuant to Article 75 of the Schengen Implementing Convention).

City of Frankfurt (Drogenreferat)
The City of Frankfurt recognizes the importance of safe, regulated access to medical cannabis. The municipal Drogenreferat (Drug Policy Coordination Office) is actively involved in organizing medical training, establishing regional networks for physicians, and providing neutral information to help integrate medical cannabis therapies into standard local healthcare practices.
